The Home-Based Primary Care Physician, in partnership with the Medical Directors and Program Managers of assigned services, is responsible for the program’s outpatient geriatric care at the patients’ homes.
This role includes the following responsibilities, in addition to adhering to the Federal, National, and Local regulatory, policy, and accrediting standards that govern this Health Care System.
Duties
Provides clinical input and oversight on patients’ treatment plans. Being easily accessible to team members for collaboration when medical or other issues arise. – Ensure that care is administered following acknowledged practices and evidence-based guidelines. Ensure timely and appropriate care delivery. Oversee delegated care activities by Nurse Practitioners, Physician Assistants, Pharm D’s, and others with a “scope of practice
- Completes patient visit documentation in CPRS with appropriate interactions in a timely way.
Ensure adequate consultation and implementation of consultant recommendations. - Address infection control issues such as skin testing, immunization, and prevention.
The Practitioner will provide cross-coverage to the various clinical GEC services within the Professional Standards’ approved and suggested privilege levels.
